Contacting the surgery to make a complaint
Our staff are here to help you. Most problems can be sorted out quickly and easily with the person concerned, often at the time they arise.
Please consider talking to us face to face: We like hearing your views and ideas, and if you think something has gone wrong we can usually sort it out there and then.
Comments & Suggestions Form: For general informal feedback. Complete the form in the surgery and pop it in the box or use the automated feedback form on our website here.
Formal Complaints: Please use the form below when you feel talking to us has not satisfied you or you believe is of a serious nature, including clinical care.
NHS Review our Service: Please use this for general reviews of the service you wish others to read. This is on the NHS website click here. Leaving a negative review does not always lead to a resolution to your issue, as we may not be supplied with the information we need to do this for you. Please give us this opportunity first by contacting the practice.
